- Neuropathic pain: Amitriptyline / Gabapentin / pregabalin; also SNRIs e.g. duloxetine.
–Restless leg syndrome: Requip/ Ropinirole
-Duloxetine is used by Dr. Toby for a patient who doesn’t do well with Gabapentin. Can use both. - Anti-inflammatory: Mobic (systemic) and Voltaren Gel(topical). Ketorolac = Toradol; Meloxicam=Mobic
- Muscle relaxer: Baclofen, Flexeril (Cyclobenzaprine)
- Lidocaine patch
Opioids (cause itchiness from histamine release. ) - Norco or Percocet 10-325
- Morphine 30mg ER po TID (for very bad pain)
- Fentanyl patch 25 (Dr. Toby recommended to a patient who failed Norco and Morphine TX–these meds didn’t work)
- Methadone (Let pain management prescribe it)
Injections - Steroid injections by PCP or at Pain Clinic.
Others - Back brace
- Physical therapy
- TENS Unit — insurance only pays it for knee pain or shoulder pain.
- Pain management referral
