Antibiotics ideal for intravenous-to-oral (IV-to-PO) switch programs include:
- clindamycin,
- metronidazole,
- trimethoprim-sulfamethoxazole,
- fluconazole, itraconazole, voriconazole,
- doxycycline, minocycline,
- Fluoroquinolones: levofloxacin, moxifloxacin, Ofloxacin,
- linezolid
Also, the following can be used to switch from IV to Po.
Cephalosporins
- Cefuroxime, cefuroxime axetil, and cefetamet pivoxil
- Ceftriaxone and ceftibuten
- Ceftriaxone and Cefdinir
Macrolides
Azithromycin and clarithromycin can be used when switching from IV to PO antibiotics.
Other antibiotics
Ertapenem can also be taken po.
