GI Cocktail or Gastric Cocktail
GI cocktail: Give to a patient who is symptomatic during the visit. It’s both diagnostic and therapeutic. It works for GERD, stomach ulcers, and dyspepsia. Give and observe the patient for about 15 minutes. If the patient responds to it, you know the source of their pain is the stomach. This is helpful because GERD or ulcer pain can mimic MI pain. Responding to it with almost complete resolution of the pain indicates a gastric source for the pain.
There are many different concoctions of the GI cocktail.
GI Cocktail # 1
We use GI cocktail #1 in our clinic.
GI cocktail: Maalox______ Lidocaine______ Donnatal________
[Mixture is 20ml-20ml-10ml for a total of 50ml. You can also do 10-10-10 for a total of 30ml].
Donnatal = Atropine / Hyoscyamine / Phenobarbital / Scopolamine
It is a mixture of:
- Liquid antacid,
- Viscous lidocaine, and an
- anticholinergic
GI Cocktail # 2
This one is available on formulary in our hospital.
Antacid-Simethicone / Viscous Lidocaine – 30 ml mixture of these three.
