Sample discharge A/P for a patient with diabetic foot ulcer-cellulitis.
Patient is s/p I&D to the distal third toe as well as nail avulsion. The patient is s/p I&D to the distal 3rd toe as well as nail avulsion. I flushed out the abscess with normal saline and repacked the abscess with iodoform gauze and soaked in saline and dressed the area with antibiotic ointment and redressed the digit and foot with dressing and an Ace bandage. We will order a postoperative shoe for the patient to leave the hospital and to allow for accommodation for the dressing. The patient should continue Augmentin for 10-14 days and will need to follow up with me in the wound center for followup. I spent time explaining to the patient how to do the dressing changes. He will remove the dressing that I was placed tomorrow and remove the packing.
He will soak his feet in Epson salt water bath and do this twice daily in lukewarm water and patient will dress the foot with antibiotic ointment and sterile dressing. Patient will continue this dressing change routine until he sees me in the wound center next week. Patient says he understands how to do the dressing changes and has Epson salt at home and we have dispensed a pack of antibiotic ointment and gauze to the patient in order for him to continue the dressing changes.
