For exercise stress test, the standard Bruce (in which treadmill speed and incline is increased every three minutes) or the modified Bruce (which begins without treadmill incline) can be used.
Hold nitrates/CCB/BB/ ranolazine if you are doing the exercise stress test to diagnose CAD. If you are trying to test if the patient is ischemic on meds, obviously let the patient take the medications.
